Voluntary Self-Identification Survey

CTC is a federal government contractor. As a matter of company policy, as well as applicable law, we are required to keep records and perform certain analyses of our applicant pool by race, ethnicity and gender. Since such analyses are only possible if we know the EEO profile of our applicants, a Voluntary Self-Identification Survey form is available online for those willing to provide their profiles. Please complete and submit this form promptly (click on the "submit" button for electronic submittal or print and mail the form).

The information that applicants provide does not affect their prospects for employment and is treated confidentially. This information is very important to CTC. For any statistical analysis to be meaningful, we must have information on as many applicants as possible. It is just as important for CTC to collect this information from men and non-minorities as it is to obtain it from women and minority-group members.

We acknowledge that some applicants may find this request intrusive. However, the government requires us to keep records and perform the analyses referenced above. Therefore, your cooperation will allow us to maintain accurate records.

In addition, information on county and state of residence, as well as on how you learned about the vacancy for which you applied, will assist us in our recruitment efforts. Please note that the categories on the survey form are those used by the U.S. Bureau of Census and the U.S. Department of Labor, and they are the only options currently available to you for federal reporting purposes.
